Who we are

Proactive Health Care Services is a health industry that provides holistic healthcare, such as Primary Care, Behavioral and Mental Health Services, and Residential Services Agency. We take the three sub-divisional concepts and merge them into a bigger entity. We have the understanding that our population needs at least one of the services or all three. We provide one-stop services for all healthcare needs. We provide services for all ages.

Our office hours are Monday through Friday, 9:00 am through 5 p.m.

Mission Statement

Proactive Health Care Services is a healthcare agency dedicated to providing compassionate and Holistic care to patients of all ages. Our goal is to improve our clients' and patients’ quality of life by preventing the progression of diseases and injuries and providing high-quality services that exceed industry standards.

Vision Statement

We aim to promote wellness and disease prevention by providing excellent healthcare services within our diverse communities, meeting patients' needs, and increasing access to high-quality care for everyone.
